Open the field, then Nutrient plan
At the bottom of the plan is Soil samples, every sample on record with its lab and date.

Add sample
Tap Add sample and enter the date, the lab, and the indices as the report gives them: P, K and Mg as index numbers, pH as a number. Then save.

Read the plan again
The plan is recalculated from the new sample, including the P, K and Mg recommendation and the lime needed to reach the target pH.
